Chapter 1033: Chapter 852: Which Immortal is This?_2 Chapter 1033: Chapter 852: Which Immortal is This?_2 This patient was originally advocated by Director Long for ex-vivo resection, as the tumor was too tricky to be removed by traditional thods.
But Dr. Liao, wanting to show off his skills, thought the risks of ex-vivo resection were too high and that the surgery would take too long, so he considered the traditional surgical thod safer and more controllable.
At the ti, there wasn’t enough blood prepared for transfusion. Director Liao, who had been performing this surgery without issues for ten years, believed he was highly skilled. When it ca to preparing blood, the Blood Transfusion Departnt had said that at most 4 units were available and it would take two days to provide more. Director Liao couldn’t wait; if it was 4 units, then 4 units it would be. Normally, there wouldn’t be any problems with this surgery.
Unexpectedly, when separating the tumor on the operating table, the blood vessels were inadvertently cut, causing a gush of blood to spurt out.
The urgent issue was that the blood vessel had retracted behind the pancreas, so it was impossible to locate where it had turned to.
“How could 4 units be enough, are you kidding ? Add 6 more units and request another 2,000 milliliters of plasma; otherwise, we won’t be able to hold on. Call the blood bank, hurry up with the blood reinforcents!” Director Long had a temper in the operating room, but he never cursed anyone.
Now that Director Liao had turned the surgery into such a ss, and Director Long had co to save the day without hesitation or reprimanding Director Liao even once since his arrival.
The patrolling nurse had already urged the blood bank many tis, but it was to no avail; they also couldn’t procure blood. Sotis, when resources at the blood bank were tight, nobody could do anything about it.
The patrolling nurse had no choice but to call the blood bank again and put it on speakerphone: “Director Mi, this is Pancreatic Surgery. Director Long will speak with you.”
“Old Mi! I need you to save a life, the bleeding is so massive; how can 4 units be enough? Add another 6 units imdiately and 10 more units after that; we can’t conclude the surgery with less than 20 units.”
“Oh Old Long, we’re helpless, too; the people at the blood station have been prodded so much they’re about to curse; O-type blood is tight as it is, and you should have anticipated a surge in demand like this—”
“Bullshit is useless, do a favor, hurry up with the reinforcents, I’ll thank you later.”
“I’ll do my best—”
If it were permissible, the Blood Transfusion Departnt wouldn’t make things difficult for doctors at this ti; Director Long understood this principle. Although he was irritable, he remained sufficiently rational, and despite speaking loudly as if he were causing a big scene, he actually hadn’t quarreled with the Blood Transfusion Departnt.
Lacking sufficient blood, Director Long’s confidence was undermined, but as an experienced general, he kept his cool and carefully searched for the blood vessels behind the liver and pancreas. Nonetheless, the situation was maddening; the tumor was situated behind, making it impossible to flip it over without removing the liver and pancreas from the body.
However, this had not been discussed before the surgery, and to suddenly broach this subject now would surely not be understood by the family. Yet even if they couldn’t understand, it was necessary to have that discussion.
“Xiao Liao, if it cos down to it, we’ll have to do an ex-vivo resection and autotransplantation. Go communicate with the family and have them sign again,” said Director Long to Director Liao.
Director Liao had never had such a mishap in surgery before and was now sowhat stunned. Director Long said to him, “What’s there to be afraid of? Go on, I’ve got this covered here. You go talk; Xiao Lu, you accompany Director Liao.”
Dr. Lv was more eloquent, and Director Liao, not being articulate, was overtly nervous at this mont, which ant he definitely wouldn’t articulate well. Now was the ti for soone with strong communication skills to help.
“Xiao Lu, you take care of the communication; just have Director Liao stand by,” said Director Long.
Although Director Long was anxious, he was clear-headed and knew that it was completely inappropriate for Dr. Liao to handle the communication now.
There was still no news from the Blood Transfusion Departnt, and the patient’s blood pressure kept dropping. The odds looked grim, and although doctors must inevitably face patient deaths, it would still be incredibly nerve-wracking for the chief surgeon involved in such a situation.
Director Liao was visibly tense and distressed now.
“Perk up, there’s nothing to fear; go with Xiao Lu. Nurse, hand the aortic clamp; I need to block the aorta,” ordered Director Long, managing tasks while continuing the surgery.
A non-damaging aortic clamp was passed over, and Director Long perford a blockage on the aorta. Even with the aorta blocked, there was a limited window to locate the blood vessel, and failing to find it ant it was all in vain.
“I must be cursed today,” cursed Director Long.
Just then, a deputy director from the Intervention Departnt arrived to assist, “Director Long, do we need to perform an angiography for embolization?”
“Yes, the vessels can’t be found right now,” said Director Long, multitasking.
The doctor from the Intervention Departnt imdiately familiarized themselves with the case details, particularly the imaging, as they needed to be prepared for the intervention shortly.
“This setup won’t do; we need to move you. Why don’t we have a hybrid operating room?” observed the doctor from the Intervention Departnt, noticing that the current location was unsuitable for intervention.
Yang Ping stood before the lightbox, scrutinizing the patient’s enhanced CT scans and the angiographic images of the tumor. Before the surgery, the patient had undergone interventional treatnt, seemingly through embolization, to reduce the size of the tumor and block so vessels to minimize intraoperative bleeding.
Professor Liang was also observing, but as an Orthopedics Professor, he was not very skilled at interpreting CT images from the General Surgery Departnt.
Having reviewed all the images brought into the Operating Room, Yang Ping had a complete understanding of the relationship between the tumor and the liver, gallbladder, pancreas, and spleen, as well as their blood vessel distribution. He already had a three-dinsional anatomy diagram in his mind.
“This blood vessel is prone to injury, and it’s very likely that this particular vessel was damaged. If it’s this one, the location is very deep, and we can’t find it without lifting the liver out. We could proceed without ex-vivo, but we would have to use vascular clamps blindly,” Yang Ping said, pointing to the blood vessel angiography on the screen.
Upon hearing this, Professor Liang had complete faith in Yang Ping’s words. In such a critical mont, Professor Liang imdiately spoke to Director Long, “Director Long, let Professor Yang help you out; he’s familiar with this.”
Director Long was startled—who’s this? Is he capable?
But this was Professor Liang’s suggestion, and whether he believed it or not, he was inclined to trust. Thus, after a montary pause, he imdiately said, “Scrub in and step up on the stage!”
“Xiao Yang, go help,” Professor Liang urged without hesitation.
Yang Ping imdiately went to scrub in, and shortly after, he returned, dressed and gloved, ready to step up to the Operating Table.
“Let do it!”
Upon stepping up to the Operating Table, Yang Ping appeared like a general entering the battlefield, projecting a very strong presence. Director Long was startled, but since it was Professor Liang who had invited Yang Ping on stage, and Professor Liang was an Academician, it couldn’t be a mistake.
“You do it; the bleeding point is likely to be around here,” Director Long said, handing over the chief surgeon position.
Yang Ping took his stance, “Large curve pliers! Two of them!”
After receiving the two large curve pliers from the Nurse, he gently maneuvered them around the top and bottom of the liver, reaching behind it. With minimal movent of the vascular clamps, the two pliers gently opened and closed, then Yang Ping slowly withdrew them.
“Director Long, clamp the vascular clamps!” Yang Ping called to Director Long.
Director Long, still not fully comprehending, took a closer look and saw two vascular clamps, each clamping an end of a blood vessel. In other words, both ends of the vessel were now under his control.
The sight nearly brought tears of joy to Director Long’s eyes. He imdiately asked a Nurse for vascular clamps, then clamped down hard on both ends of the vessel and stitched them up firmly.
After suturing, Yang Ping released the aortic clamp, and as the Suction Device cleared the blood accumulated inside, there was no more significant bleeding in the surgical area.
Indeed, there was no more bleeding, and everyone was on tiptoes, trying to get a glimpse.
After bustling for so long, Yang Ping had resolved the issue in just a few minutes. Who was this extraordinary person? Director Long asked Professor Liang, “Old Liang, who’s the deity you’ve recomnded?”
Indeed, which deity was this? There were doctors present who did not recognize Yang Ping, as they were not from the sa departnt.
“Orthopedics’ independently appointed expert consultant Yang Ping!”
Yang Ping! The renowned Yang Ping. Director Long had only heard the na but had never t the man. Now, he was shocked to see him in person in such a manner.
“So it’s Yang God, no wonder he’s so formidable. I’m sincerely convinced; I’ll treat you to dinner tonight,” Director Long said, expressing his gratitude.
Doctor Lv and Director Liao returned breathlessly, “The family has agreed to our plan.”
“No need for that now, the bleeding has stopped, and it shouldn’t be a big problem. Hurry and have the Blood Transfusion Departnt bring so blood over,” Director Long said, after inspecting the surgical area and found no further bleeding.
“It was all you, Director Long!”
Even Director Liao, who was usually reticent, couldn’t help but offer his complints.
“It wasn’t my rit. I also struggled without success for quite so ti. If we are to thank anyone, it should be Professor Yang from Orthopedics, who just ca to our aid,” Director Long said, being a forthright person.
Professor Yang, in Xiehe Hospital everybody knows who you are. Everyone has heard the na, though so may not have t you in person.
