In order to further validate the hypothesis, the expansion of Yang Ping’s "New Plan" was launched under ethical green lights in multiple countries. As the number of volunteers increased, the first adverse reaction was reported.

The patient was a forty-two-year-old Swedish male Lynch Syndro carrier, identified as P-009. Seventy-two hours after receiving the first infusion of a personalized modulator, at only one-fifth of Alina’s initial dose, he developed a high fever, systemic rash, and a sharp increase in liver function indicators. After ergency hospitalization, infection and other drug factors were ruled out, pointing to a serious immune-related adverse reaction.

"It is the early manifestation of a cytokine storm," the chief researcher responsible for the center reported in an ergency video conference, with a heavy tone, "although controlled with steroids and biologics, without life-threatening danger, we had to withdraw him from the study. Biopsy showed a unique pattern of TIM expression in his intestinal tract, with nurous under-considered splice variants present."

The eting room fell silent; this is a hurdle inevitably encountered in large-scale clinical trials, but when it truly happens, especially when "system modulation" is highly anticipated and seen as a milder and safer model, the impact of the first serious adverse reaction is particularly strong.

"Our modulator is essentially a kind of exogenous protein, and moreover, an artificially modified exogenous protein. Any artificially manufactured drug or protein factor inherently has flaws, especially for the first-generation K Factors used in Lynch Syndro." Yang Ping broke the silence, his voice calm, "It binds TIM, altering its conformation, which in itself could be recognized as a new antigen by the immune system, particularly in individuals with special TIM variants. We designed it to avoid known strong immunogenic sequences, but the data basis for personalized design is still too thin to exhaust all variants. Accumulating data is also one of our experint’s goals."

He pulled up all baseline data of P-009, comparing it side by side with Alina’s. "Look here, Alina’s TIM-F variant primarily concentrates in two ring-like structures in the extracellular region, where our modulator is designed to bind gently. Whereas P-009’s variant has an additional glycosylation site near the transmbrane region, this site perhaps influenced the overall conformation post-binding, exposing previously hidden immune epitopes."

Lu Xiaolu raised a critical question: "Is this an isolated case, or does it imply a blind spot in our general strategy for personalized design? Do we need to conduct comprehensive immunogenicity predictions for each patient anew? That would increase costs and ti exponentially."

"Perhaps we need a different approach," Tang Shun gazed at the data, speaking slowly, "not to pursue absolute non-immunogenicity, which may be unrealistic. Instead, like immunotherapy, learn to manage immune responses. Our modulator triggered an undue strong immune attack, indicating the modulation signal itself was misinterpreted by the immune system. Can we design a buffering or guiding system, allowing the modulation signal to be delivered more gently, or synchronously provide trace amounts of immune modulation signals, guiding the immune system to recognize it as a friend rather than a foe? While improving our immune managent capability, we iterate the related K Factors, constantly refining them in the iteration to reduce or even eliminate such adverse immune reactions."

After team discussions, they began conceiving a second-generation modulation strategy: no longer a singular "modulator molecule," but an "intelligent modulation system," which may include the main regulatory proteins, delivery carriers, such as nanoparticles responsive to specific microenvironnts, and synergistic components for regulating the immune microenvironnt. Complexity would greatly increase, but perhaps safety could also be significantly enhanced.

The P-009 incident was swiftly and transparently reported to all trial centers and regulatory agencies. Unexpectedly, the number of participants who withdrew was minimal. Many expressed in feedback their understanding of the risks of cutting-edge exploration and willingness to contribute data for "safer future therapies."

Another reason is that traditional treatnt options are truly abysmal. The risk of Yang Ping’s new therapy is nothing in comparison; even with such risks, both the effect and safety far exceed traditional therapies.

This incident and its handling were reported by the British dical Journal as a case of "transparency and trust-building in high-risk pioneering clinical trials." Science, amidst setbacks, instead highlights its rational and responsible undertone.

The craze for the Inner Canon of Huangdi has not yet subsided and seems to be growing ever more intense.

A Silicon Valley startup launched a health monitoring and lifestyle guidance app nad "Vital Flow." Its selling point is not directly quoting the Inner Canon of Huangdi but claims its core algorithm is "inspired by the traditional Eastern holistic dicine wisdom," combining data from wearable devices bundled with the app, such as heart rate variability, sleep architecture, galvanic skin activity, activity levels, and the user’s simple daily logs of symptoms, emotions, and diet, using machine learning to generate personalized "system status daily reports" and "fine-tuning recomndations" for users.

The "daily report" does not provide diagnoses like "You have high blood pressure," but displays suggestions such as "Today’s system is inclined towards a ’hyperactive’ state, recomnd increasing gentle walking by 30 minutes, reducing spicy food at dinner," "The past week’s ’recovery’ index was relatively low, possibly related to poor sleep continuity, try starting the pre-sleep routine 15 minutes earlier," etc.

These recomndations appear like an upgraded version of a life coach, but the logic behind them attempts to mimic a simplified "system balance" model. The company hired several dical-background scientists and an expert with knowledge in traditional Chinese dicine as consultants.
