Basically, applying was enough to participate, since the competition was 'entertainnt' after all.
Even though the intention was for entertainnt, this was ultimately a formal national-level competition with official backing. Achieving good results here would make one stand out, at least within the city, if not the entire province, right?
There aren't that many patients in the hospitals of prefecture-level cities, so why wouldn't all the hand surgery departnts rush to compete for them?
With more people, the organizers couldn't handle it, so they had to adopt a recomndation system. The hand surgery professional committees in each provincial capital recomnded so people to participate.
Controlling the number of participants also made the competition more qualitative and easier to manage.
