Capítulo 1839: Chapter 1288: The Real Patient (Part 2)
This candid confession changed the atmosphere in the eting Room. Director Sun stopped joking, and Professor Chen nodded seriously.
Professor Chen spoke, “Director Zhao’s dilemma is very real. In traditional dical fraworks, unless there is a clear psychiatric indication, pediatricians are hesitant to abruptly involve the psychological departnt in the diagnosis and treatnt of a child with somatic symptoms. The biggest characteristic of this case is that the presentation lies entirely within the realm of non-patient somatic symptoms, and the psychological problem of the patient is deeply hidden, separating the real patient from the so-called patient.”
Yang Ping sat back down, “This is exactly what I want to stress. Solving this case is not about which hospital is more advanced, but about the thought process and collaborative model in diagnosis and treatnt. Director Zhao, being able to have doubts and choose to transfer the case despite lacking evidence, requires imnse professional insight and courage. Without the extensive preliminary investigations by Director Zhao, we might not have been able to reach a diagnosis. None of the diseases we considered fit, which in itself served as a clear guide.”
He opened the projector, “Now we need to face how to properly handle this confird diagnosis. We need collaboration between the pediatrics departnt, ntal health departnt, and social work departnt, and possibly even legal advisors and police involvent. Communication must be extrely cautious and cannot be simply about exposing the truth.”
“But how do we present the toxicological evidence?” asked Song Zimo, “This is the most sensitive part, and this evidence was discovered at our Sanbo. Will the family think that Affiliated Hospital No. 1 didn’t notice anything for two years, and then suddenly Sanbo finds the poison?”
This question struck the core, and for a mont, all eyes were on Yang Ping.
Yang Ping had already thought about this difficult issue, “During communication, we will downplay the boundaries between hospitals, emphasizing that it is a continuous dical process. We will say that the comprehensive initial investigations at Affiliated Hospital No. 1 laid the foundation for finally locking in a direction, and Sanbo’s observational environnt and cross-disciplinary model happened to provide conditions for discovering this special behavior pattern. As for the toxicological evidence, we will present it as an anomaly discovered in a thorough review of the entire diagnostic and treatnt process, rather than as newfound evidence at Sanbo.”
He looked at Director Zhao and Professor Chen, “How about I, Director Zhao, and Professor Chen take the lead in communication? Director Zhao represents the initial diagnostic and treatnt team, I represent the current team, and Professor Chen provides a psychological perspective. We form a continuous, unified professional front to avoid giving the family the impression that there is inconsistency between hospital conclusions. Whether police involvent is needed depends on how things develop, but we should record this matter in advance.”
Director Zhao was sowhat surprised but then showed gratitude in his eyes. Yang Ping’s arrangent not only protected the professional reputation of Affiliated Hospital No. 1 but also reflected a rare broad-mindedness. The essence of dicine is to solve problems, not to scramble for credit.
“I agree,” Director Zhao said solemnly, “As Lele’s primary doctor in the initial stage, I have a responsibility to participate in this difficult communication process.”
Director Sun stood up at this point, walked over to Yang Ping, and extended his hand, “Professor Yang, I apologize for my joke earlier. You not only have superb dical skills but also the broad mind and wisdom. I will personally deliver the consolidated volu and the single volu when I return.”
The communication took place the next morning. Ms. Qiao visibly hesitated when she saw Director Zhao, “Director Zhao, you ca too?”
Director Zhao nodded gently, “Lele’s situation has been on my mind all the ti. Professor Yang invited to participate in this important discussion, and I’m very grateful. Our treatnt of Lele is a continuous process.”
This communication was more difficult than expected.
When Yang Ping gradually presented observational evidence, Ms. Qiao reacted intensely, “Are you implying that I’m harming my son? Director Zhao, Lele has been at Affiliated Hospital No. 1 for two years, you know us best, is that the kind of person I am?”
Director Zhao was silent for a few seconds before speaking slowly, “Ms. Qiao, over these past two years, I have seen everything you’ve done for Lele. No mother would intentionally harm her own child. But sotis, excessive love and anxiety, coupled with past trauma, can lead to changes in behavior that even the person themselves cannot understand.” He opened Lele’s case file at Affiliated Hospital No. 1, “Do you rember? Last March, when Lele was hospitalized at Affiliated Hospital No. 1, we did routine checks on stomach contents after three episodes of vomiting. We didn’t conduct a toxicology screen at that ti; if we had thought more broadly…”
He didn’t finish the sentence, but the implication left Ms. Qiao frozen.
Professor Chen tily interjected, analyzing from the perspective of psychological trauma the possible connection between Ms. Qiao’s childhood experiences and her current behavior. When discussing her experience of caring for her mother with cancer starting at age eight, Ms. Qiao’s defenses finally began to crumble.
“Sotis… I feel that only when Lele is sick do I feel alive, feel needed…” she cried uncontrollably, “During those two years at Affiliated Hospital No. 1, every ti the doctors discussed his illness, every ti a nurse told ‘you are truly a great mother,’ I felt that was my only value.”
Director Zhao’s eyes reddened slightly. He now fully understood why this case was so tricky. This mother needed not only her child’s treatnt but also healing for her own inner trauma. So use childhood to heal a lifeti, while others use a lifeti to heal childhood.
After the communication, the support group imdiately initiated the intervention plan. Director Zhao’s participation increased the father’s acceptance of the plan, as he trusted this veteran expert who had been involved for two years. The plan continued with Yang Ping’s design: Lele would temporarily be cared for by the father’s side while Ms. Qiao began systematic psychological treatnt and family therapy was conducted simultaneously.
